This final project presented us with a real-world scenario to implement the program planning and evaluation skills and knowledge we gained from the course.


Purpose of Evaluation: To track the reduction of Alta Bates patients’ high blood pressure levels after the implementation of the hypertension management program. The findings of this program after implementation will be used by Alta Bates’ medical staff and administration to better understand their patient population and allow them to better coordinate their care when it comes to hypertension management.

This project was divided into three deliverables:

  • Section 1: Background (of the program) and Logic Model implemented for evaluation

  • Section 2: Methodology (Goals, Objectives, and Indicators)

  • Final Comprehensive Evaluation Plan Outline: includes Section 1 & 2, Data Analysis Plan, Sampling Plan, and anticipated implementation challenges
